Initial Evaluation
Typically, 1.5 hours, the initial interview will cover as sensitively as possible your reasons for seeking psychiatric care; past psychiatric, medical, developmental, family history, and previous therapies. We’ll also go over diet, lifestyle/exercise, environmental factors, and psychological stressors. You’re encouraged to bring any records you may have, such as testing done, lab work, medical records, and all current prescriptions bottles but these are not necessary to perform the initial evaluation. Because I do a longer initial visit, we cover more ground and can produce a more comprehensive treatment plan. If needed I consult with other physicians and therapists involved as part of the initial fee.
Fee $320
Psychiatric Follow up Visits
These can be either be 20-30 minutes, or 45-60 minutes, depending on the nature of your treatment. This includes managing medications and supplements, hypnotherapy, and addressing medical, social, environmental and psychological issues.
Fee $150 (20-30 minutes), $240 (45-60 minutes), $55 (for each additional 15 minutes)

CANCELLATIONS
Please notify me as soon as you know you must miss a scheduled appointment. Cancellations disrupt my schedule and your progress in treatment; I will not be able to continue your treatment if you frequently cancel.
If an appointment is cancelled with at least 24 hours notice, there is no charge incurred. A first time cancellation within 24 hours of the scheduled appointment will not be penalized. A second cancellation within 24 hours of the scheduled appointment will result in a fee equivalent to half the amount of the normal visit rate. A third cancellation within 24 hours of the scheduled appointment will result in a fee of the full normal visit rate. Exceptions will be dealt with on a case by case basis.

Questions or complaints about any aspect of your care are welcomed and encouraged; if you are unhappy about something, I may be able to do something about it, but only if you tell me.
PAYMENT POLICY
Payment is full is due at the time of each appointment, unless other arrangements have been made. I do not participate in any managed care plan.
Please advise me if you suffer financial hardship so that we may plan in a manner fair to each of us for you continuing care.
